Ageing Support (CHC43015) — RPL Evidence Portfolio Guide

Certificate IV in Ageing Support

Evidence portfolio guide for experienced aged care workers stepping up to team leader, care coordination and CHC43015 recognition.

Last updated 7 September 2026

Who this guide is for

  • Aged care workers moving into senior carer, team leader or coordinator roles
  • CHC33021 holders with residential or home care experience
  • Home care package workers coordinating services and liaising with families
  • Candidates preparing a VETASSESS skills assessment in an aged care occupation

1. Visual evidence guidelines (photos & videos)

To satisfy independent RTO assessment criteria, submit 8 de-identified workplace photos and 4 video clips (about 60 seconds each).

Privacy comes first: nothing that identifies a resident or client — use simulated or training-room demonstrations with de-identified notes and written employer consent.

Option A — Assessment and care planning

Mandatory
  • Initial and review assessments
  • Contributing to or revising a care plan
  • Goal-setting with the client and family
  • Documenting a change and the resulting plan change

Option B — Complex care and clinical awareness

Mandatory
  • Dementia support and behaviour support
  • Palliative and end-of-life care within scope
  • Recognising deterioration and escalating
  • Falls, wound and pressure-area risk management

Option C — Leading and supporting a team

3 to 4 tasks
  • Allocating tasks and running handover
  • Coaching or orienting a new worker
  • Rostering and resolving staffing issues
  • Leading a team discussion

Option D — Rights, quality and compliance

3 to 4 tasks
  • Applying the Aged Care Quality Standards
  • Handling a complaint or feedback
  • Reportable incident scheme awareness
  • Advocating choice and dignity of risk

Option E — Coordination and community

3 to 4 tasks
  • Arranging allied health or external services
  • Liaising with families, guardians or case managers
  • Supporting community participation
  • Managing a home care package budget

These task counts are our standard preparation guidance. Your assessing RTO may set different requirements — we confirm them before you start collecting.

2. Acceptable vs unacceptable visual evidence

Acceptable evidence (do this)

Unacceptable evidence (avoid this)

Supervisor report describing decisions you made, not just dates worked
Any resident name, room number or date of birth left visible
De-identified care plans plus written employer consent
Reference confirming dates only, with no judgement or decisions
Escalation examples showing your reasoning, not just following instructions
Support-worker-level tasks presented as coordination
Evidence of leading others, not only doing the tasks yourself
Acting-up claims with no record to support them
Payslips plus position description for any senior or acting role
No example linked to the Aged Care Quality Standards
Current screening checks and training records in your name
Expired checks or training records in someone else's name

3. Supporting documents checklist

  • Photo identification (passport or Australian driver licence)
  • Résumé showing your level of responsibility, not just job titles
  • Position descriptions for any senior roles
  • Third-party report from a qualified supervisor or clinical manager
  • Payslips, rosters and PAYG covering the claimed period
  • NDIS Worker Screening and police check
  • Current first aid and CPR certificates
  • Dementia, palliative, medication and manual handling training records
  • Existing CHC33021 qualification if held

4. How to organise and submit your portfolio

  1. 1. Confirm consent and privacy rules

Get written employer consent and confirm what may be shown — expect simulated demonstrations only.

  2. 2. Choose examples that show decisions

Pick cases where you assessed, planned, coordinated or escalated — not routine tasks alone.

  3. 3. Write a short scenario per example

Two to three de-identified sentences per example: the situation, what you decided, and the outcome.

  4. 4. Organise by option with an index

One folder per option plus an index page mapping each file to its task.

  5. 5. Keep documents in their own folder

Identification, references, pay records and training certificates sit separately from visual evidence.

  6. 6. Prepare for a competency conversation

Be ready to talk through judgement calls, escalation decisions and quality standards in detail.

5. Why portfolios get sent back

  • Support-worker-level tasks only, with no planning or coordination
  • Client identity left visible in any file
  • No third-party report from a qualified supervisor
  • Leadership claimed with no record to support it
  • No example linked to the Aged Care Quality Standards
  • Claimed period unsupported by pay records

6. Questions we get asked

I do the senior work but my title is still support worker. Does it count?

Yes, if your supervisor confirms it in a third-party report describing the actual responsibilities. Title matters less than documented practice — though for a migration assessment the title and pay level matter a great deal, so tell us if that is your goal.

Can I use a care plan I wrote?

Only fully de-identified and only with your employer's written consent. Never remove documents from a workplace system without permission.

How much experience do assessors expect?

As a guide, around three years in aged care with at least twelve months carrying senior responsibility. Less than that and we will usually recommend a gap-training plan first.

Who makes the final decision?

An independent registered training organisation. Skills Connect prepares and reviews your evidence; we do not assess or issue the qualification.

Not sure your evidence is strong enough?

Take the free skills check, or book a call and we'll review what you already have before you spend weeks collecting the wrong things.

See the full Ageing Support pathway page for qualification, licence and visa outcomes.

Skills Connect (EDUTEK PTY LTD) prepares and reviews RPL evidence. Assessment and certification are carried out by an independent registered training organisation or assessing authority.
